We require that the dancers PARENT/ GUARDIAN/ reads and agrees to the following waiver: The MEMBER/ PARENT/ GUARDIAN waives and releases Studio G Dance Academy; any program; and it’s employees from any and all claims for personal injury arising during the participation in any program, and/ or while waiting unsupervised before and after class. The MEMBER/ PARENT/ GUARDIAN has read the website/ brochure/ registration details and accepts all policies posted on studiogdanceacademy.com.

FULL REGISTRATION REQUIRED BY MONDAY SEPTEMBER 10TH 2018 PRIOR TO COMMENCEMENT OF CLASSES *

*Your Dancer is considered fully registered once the following fees have been paid: $28 registration fee/ $50 costume deposits for each recital class/ specialty classes/ Month of September tuition fully paid. The June payment is payable June 1st. Any outstanding balances paid after June 1st will incur $40 late fee. Your dancer will not be able to participate in the Year End show if any outstanding fees are not paid up to date. *Overriding your chosen payment plan to pay off the fees early is also accepted.
