Participation Waiver *

We require that the dancers PARENT/ GUARDIAN/ reads and agrees to the following waiver: The MEMBER/ PARENT/ GUARDIAN waives and releases Studio G Dance Academy; any program; and it’s employees from any and all claims for personal injury arising during the participation in any program, and/ or while waiting unsupervised before and after class. Off-sight Activities: I give permission for my child to attend off-sight activities. Also, I waive and release Studio G Dance Academy Ltd, its employees, agents and volunteers, from any and all claims for personal injury, loss, and causes of action arising during the participation of off-sight activities, and travel to and from off-sight events/programs/ activities. *

Off-sight locations will be within walking distance from the studio, such as play-grounds, sport fields, trails and nature parks. Parents will be notified in advance with a location schedule. Trips to off-sight locations are weather permitted.
